Patient Liaison Nurse


We are currently seeking to recruit an experienced and highly motivated Registered Nurse to co-ordinate all admissions and discharges to the hospice. This is an exciting opportunity for anyone wishing to continue to develop existing skills within palliative care.  Working as part of the multi-professional team in collaboration with others to promote specialist services for the people of Bolton.

Salary £30,615 per annum

(Band 5 Senior Staff Nurse)

Full Time 37.5hrs Monday to Friday

The Role

The Patient Liaison Nurse provides a single point of contact for patients, their families, referrers and other health and social care workers, who are seeking an admission to the hospice Inpatient Unit or other hospice services.  The role requires an effective co-ordination of the hospice patient admission and discharge processes and works in collaboration with hospice clinicians, members of the multi-disciplinary team, primary and secondary care services and patients and carers.

What you need:

We are looking for an appropriately qualified Registered Nurse with significant post-qualification experience working in the Community or Inpatient Unit. Experience in palliative/ end of life care is desirable. You must be able to communicate sensitively and effectively with patients, their carers and professionals.  Excellent assessment skills are essential and experience of triaging admissions would be beneficial.  Excellent organisational skills and the ability to deal with changing priorities are also key for this role.  You will have access to a range of training and development opportunities and be welcomed as a valued member of an experienced and supportive team.
