Young Adult Transition Project Lead


 

An exciting new opportunity has arisen for an experienced professionally qualified Nurse, Occupational Therapist or Physiotherapist to join our team as a Young Adults Transition Lead. The post holder will lead on the transformational development and delivery of a transition service for young adults, working alongside the amazing team here at Bolton Hospice.  Creating a transition service of excellence from children’s hospice services and other care providers. The service will deliver outstanding care and support for young adults with life limiting conditions, enabling access to adult specialist palliative care services, through our Wellbeing Hub, Hospice@Home and Inpatient Unit.  Transition services truly make a difference to young adults and those most important to them, increasing wellbeing, life fulfilment and achievement of personal goals.  Team Bolton Hospice is friendly, motivated and committed to providing a high quality, expert specialist palliative and end of life care to the people of Bolton.

Two year fixed term contract, with potential for
a permanent post

Band 7 £40,057 - £45,839 per annum pro rata

(cost of living increase pending)

30 hours / 4 days per week (would consider full time 37.5 hours per week for the right candidate)

Flexibility may be required to meet project and service needs

The Role

The Young Adults Transition Lead will work proactively with key stakeholders, including young adults, their families and relevant health and social care providers, to design, develop, deliver and evaluate a Young Adults Transition Service.  The post holder will promote and champion the needs of young people with life limiting conditions through the development of transition pathways and services which will promote autonomy and independence. The role will involve developing and supervising a rolling caseload of young people on their transition journey into Adult Hospice Services.

This is a leadership role, designing and leading the project roll out, working together with others, including planning, delivery, milestone development; reporting and evaluation of the Young Adults Transition Service. The successful candidate will enjoy excellent stakeholder engagement throughout the project journey.  You will be supported within a multi-disciplinary team structure and also work autonomously, in a dynamic learning environment, with subject matter experts, supporting your own personal development and acquiring of new skills.

What you need:

We are seeking a candidate who has either a Degree in Nursing, Physiotherapy or Occupational Therapy and holds a Professional Registration with the relevant body, e.g. NMC/HCPC. We are confident that you will be enthusiastic and flexible, and a great team player, with some experience of working with young people in a young people related discipline, e.g. hospital services, youth work, social work etc.  Experience in successfully managing and delivering change is a key attribute, as is a passion for enabling young adults to have equitable access to specialist palliative and end of life care.  You will have access to a range of training and development opportunities and be welcomed as a valued member of an experienced and supportive team.
